| I do hereby make application for entry as a contestant in an official
Regional, State or Country Competition, preliminary to the International
Competition. I will, if selected the winner, go to the host city in order
to participate in the next phase of competition.

| I agree to abide by all rules and regulations of the Regional, State,
and International contests.

| I permit PBJ, Inc., and their licensees the right to use my photograph
for publicity purposes in connection with all contests, and if any portion
of a competition is televised live or by the use of film or video tape, I
do so permit.

| I agree that the decision of the judges is final.
|
|
| I understand that, if selected as the winner, of this competition,
that for one year my professional affairs as said winner will be under the
sole management of PBJ, Inc., and/or their licensees and that I will not
give any written or oral endorsements commercially without the expressed
permission of said same. I acknowledge that all rights for any
appearances, endorsements, speaking engagements, acting roles, etc.,
resulting from winning or being a contestant in any regional, state or
International competition affiliated with PBJ, Inc., shall belong
exclusively to same.
